ENROLLMENT PROCEDURES


Welcome to Anthony Wayne Local Schools!

All new students must be registered at the Anthony Wayne Administrative Center by a parent or legal guardian.   Registration is by appointment only.  You may schedule an appointment by calling 419-877-5377. The registration process includes the completion of enrollment forms as well as providing the required documents. Registrations will be considered incomplete,and students may not start school,until we have received all forms and documents.

You may pick up a registration packet at our Administrative Center at 9565 Bucher Road in Whitehouse prior to your appointment.  Registration forms are also available on our website, www.anthonywayneschools.org. Registration packets are not available by mail, and we will not accept faxed registration forms

In compliance with the requirements established by Ohio law, the Ohio Revised Code and the Missing Children’s Act, along with the Anthony Wayne Board of Education, please bring the following information and documentation when you enroll your child(ren) in Anthony Wayne Local Schools.

  • The child’s Official Birth Certificate from state bureau of vital statistics

  • Photo identification (parent or guardian’s driver’s license/state ID)

  • Proof of Residency - Anthony Wayne Local Schools does not have an open enrollment policy. All students must be Anthony Wayne School District residents.  Therefore, parents must provide one of the following:
    • Two current utility bills (gas, electric, water, cable TV)
    • Purchase agreement and current utility bill
    • Lease/rental agreement and a current utility bill
    • If you are building a house in the district, a copy of the Builder’s agreement and a letter from Builder stating that occupancy will take place within 90 daysfrom admission date; parent must agree to transport child to and from school until they occupy the new house. 
    • Residency Affidavit and utility bill (for guests of Anthony Wayne residents)

  • Custody Document - If child is NOT living with BOTH biological parents listed on the official birth certificate, you must provide one of the following:
    • A copy of the complete court document (divorce decree, shared parenting agreement, etc.) including the cover page with filed/date stamp, page stating legal custodial/residentialparent, and judge/magistrate’s signature page.
    • If legal custody has not been established, the petitioner has sixty (60) days from the time of enrollment to produce a signed Journal Entry of Judgment establishing a change in custody. In order for your child to be enrolled in Anthony Wayne Schools during this waiting period, you must provide a copy of the filed/date stamped motion petitioning the court for a change in custody.

  • School Records (Please bring these with you to your scheduled appointment to enroll your child)
    • Immunization records
    • Current transcript or most recent class schedule
    • The name, address, and telephone number of your child’s previous school
    • Most recent grade card
    • Special Education records (IEP/504 Plan/MFE)
