PREPARTICIPATION ATHLETIC PHYSICALS

Preparticipation physicals will be offered for Anthony Wayne students entering grades 7-12 for the 2008/2009 school year.

PLEASE NOTE:

ALL FORMS MUST BE COMPLETELY FILLED OUT AND SIGNED BY THE ATHLETE AND PARENTS PRIOR TO THE PHYSICAL – NO EXCEPTIONS. 

 
Blank physical forms may be picked up from the athletic office or training room or downloaded here.

No make-ups will be allowed; however, you may come either night if you are not available at your scheduled date/time.

Girls are asked to wear sports bras under appropriate attire for the physical exam portion of their physical.

Copies of the completed physical forms will be available upon request as no camp physicals or work permits will be completed.
